Referrals

If you have been sent to us by another physician, please tell us, so we may report to him or her promptly. 

Our doctors believe in working closely with your referring physician.  They will personally contact your doctor, so that both may actively be involved in your care.  

If you belong to an HMO and your primary care physician has referred you to our care, we will need a referral form sent to us prior to making your appointment.
