Making Appointments 

We see patients by appointment Monday through Friday from 8:00 a.m. to Noon and 1:00 pm to 4:00 p.m. Our telephone numbers are 623-584-2127 in Sun City West, 623-972-3992 in Peoria and 623-935-0247 in Goodyear. Our telephone receptionists are available Monday through Friday from 8:00 a.m. to Noon and 1:00 pm to 5:00 p.m. 

On your first visit, we will ask you to complete a brief medical history and registration form. You can fill out the MEDICAL HISTORY FORMS online, print them out on your computer and bring them in with you to the appointment. This will satisfy the medical history requirement and save you time when you come in for your first visit to the office.

Please bring your insurance cards with you and please plan to arrive thirty minutes early. We also may ask your permission to transfer some of your past dermatology records to our office. 

As a courtesy to our patients, we confirm all appointments one or two working days prior to the appointment date. If you are unable to keep your appointment, or are going to be late, please call our office as soon as possible.  This courtesy allows us to be of service to other patients. A $25.00 fee may be charged for any appointment not kept or cancelled 24 hours in advance.
